Digital COVID-19 Vaccine Record
If you are asked for your vaccine record, you can show your physical vaccine card or you can receive a digital copy of your vaccine record from the State of California.

to get a link to a QR code and digital copy of your COVID-19 vaccination record.
You will be asked to enter your first name, last name, date of birth, and the cell phone or email address associated with your vaccine record. Then you'll create a 4-digit pin number. You will be sent a link (via text or email) to retrieve your digital vaccine card and will be asked for the pin number you created.
If you have an iPhone: You can save your digital vaccine record by taking a screenshot and saving it to your camera roll. You can also save the screenshot to your iPhone files.
If you have an Android phone: You can save your digital vaccine record on Google Pay or you can save by taking a screenshot and saving it to your camera roll.
